Here are customizable information packets on therapies that you can print out for your patients with different tumor types. Please click on the drug you are interested in and the tumor type, and you will be directed to the printable action plans. Each action plan has a cover where you can add the patient’s name, the date, a list of side effects discussed, as well as key points. Throughout the action plan you can also notate side effects you want the patient to pay particular attention to.

For some tumor types, we have obtained endorsement from a major advocacy organization in the field. In those instances, you will see co-branding and additional resources related to the advocacy organization.
